Greater prevalence of cancerespecially among the aging generationand better outcomes of cancer treatment mean that more people are living with the effects of cancer treatmentThese specialized healthcare needs can be most effectively addressed by adherence to a survivorship care planSCPThe SCP coordinates monitoring for cancer recurrencemanagement of the effects of treatmentand guidelines for healthy behaviorsHoweveradherence to the SCP can be challenging for survivorsFurthermorehealthcare providers typically have limited access to information about survivor health and habitsTo provide tools that empower survivors to follow their SCP and to give healthcare providers access to more comprehensive health informationwe propose to develop a POST treatment Health Outcomes of CancerPOSTHOCplatform that incorporates mobile healthmHealthtechnologies and up to date health IT standards to manage survivor healthPOSTHOC leverages mobile and wearable technologies to reinforce the SCP by automatically tracking adherence to physical activity guidelinesassessing and monitoring symptoms through an easily accessible mobile interfaceand enabling survivors to provide timely health informationIn this wayPOSTHOC gives providers tools that extend beyond the walls of the clinicassimilate and manage patient health outcomesand empower survivors to manage their ongoing health
